Ultrasonido a pie de cama para diagnóstico y drenaje percutáneo de absceso hepático piógeno en choque séptico: Reporte de caso en entorno de recursos limitados

Bedside ultrasound for diagnosis and percutaneous drainage of pyogenic liver abscess in septic shock: Case report in a resource-limited setting

Abstract
Abstract: Point-of-care ultrasound has revolutionized medical practice, offering personalized treatment, diagnostic accuracy, continuous monitoring with follow-up interventions, and enhanced patient safety. We report the case of a 47-year-old woman admitted with abdominal pain, progressive hemodynamic deterioration, multiple organ failure, and septic shock with elevated acute-phase reactants. Point-of-care ultrasound was performed to identify the source of infection, revealing multiple liver abscesses. Given the limited resources available due to the lack of interventional radiology, drainage was performed by an intensivist trained in ultrasound using an ultraportable pocket-sized unit and a central venous catheter as the only therapeutic option for controlling the infectious focus in a life-threatening situation. The collection was drained, resulting in improvement of organ function and discharge.

Point-of-care ultrasound enabled the identification and control of the infectious focus in a patient with septic shock, becoming the primary diagnostic and therapeutic tool in a resource-limited setting. It is now common knowledge that the applicability of personalized bedside ultrasound can prevent catastrophic events in various medical emergencies.

Resumen:

La ultrasonografía en el punto de atención ha revolucionado la práctica médica, ofreciendo personalización de la terapéutica, precisión diagnóstica, monitoreo continuo con intervenciones y medidas subsecuentes, así como mayor seguridad para el paciente. Reportamos el caso de una mujer de 47 años que ingresó por dolor abdominal con progresivo deterioro hemodinámico, falla orgánica múltiple y choque séptico con reactantes de fase aguda elevados. Se realizó ecografía en el punto de atención en busca del posible foco infeccioso, evidenciándose abscesos hepáticos múltiples. Ante una situación de recursos limitados por no contar con radiología intervencionista, se realizó el drenaje por un médico intensivista con formación en ultrasonido, utilizando un equipo ultraportátil de bolsillo y un catéter venoso central como único recurso terapéutico para el control del foco infeccioso en una situación de riesgo vital inminente. Se logró drenar la colección con la consecuente mejoría de los fallos orgánicos y posterior egreso hospitalario.

El ultrasonido en el punto de atención permitió identificar y controlar el foco infeccioso en una paciente con choque séptico, convirtiéndose en la principal herramienta diagnóstica y terapéutica en un entorno de recursos limitados. En la actualidad es de conocimiento general que la aplicabilidad de la ecografía a pie de cama de forma personalizada puede prevenir eventos catastróficos en diversas emergencias médicas.
