Abstract
Orthotopic liver transplantation is one of the greatest challenges faced by the anesthesiologist, therefore, the importance of a correct pre-anesthetic assessment, in order to bring the patient to the most optimal level possible, based on the clinical characteristics that This can present as a result of the different pathophysiological changes that liver disease entails in multiple systems: neurological, pulmonary, cardiac, renal, coagulation status, fragility, and nutritional status. In addition, it will allow a correct staging of both the disease and the risks related to the surgical act, in order to be able to designate the best strategy adapted to the particular needs of each patient.
Resumen
El transplante hepático ortotópico es uno de los mas grandes retos al cual se enfrente el medico anestesiólogo, por eso, la importancia de una correcta valoración preanestésica, con la finalidad de llevar al paciente al nivel mas óptimo posible, con base a las características clínicas que este puede presentar como resultado de los diferentes cambios fisiopatológicos que conlleva la enfermedad hepática en los múltiples sistemas: neurológico, pulmonar, cardiaca, renal, estado de coagulación, fragilidad y el estado nutricional. Ademas, de que permitirá una correcta estatificación, tanto de la enfermedad como de los riesgos relacionados al acto quirúrgico, para así, poder designar la mejor estrategia adaptada a las necesidades particulares de cada pacientes.
